英文の構造

a bottle of Spanish wine and some raisins before them と the doctor smoking away は単語が並んでいるだけで、動詞がありません。なぜかというと、どちらの語句グループも文頭の I found につながっているからです。

意味を考えるときには、こんな具合に I found を補います。

（I found）the doctor smoking away

この場合の find は find●▲で、「●が▲（の状態）だと気づく」という意味です。
